NEW PATIENTS

Oasis Counseling Center is currently accepting new clients. To schedule an appointment, please give us a call at 765.453.4500. We will ask you a few questions in order to match you with a therapist in our office. Questions include basic information such as your name, date of birth, address, and phone number, as well as any insurance subscriber IDs, primary care physicians and referral sources.

We will then schedule your first appointment with a Therapist for counseling based on your needs.

At your first appointment, we will ask you to arrive at least 15 minutes early in order to fill out new patient paperwork and allow us to make a copy of your insurance card(s). For your convenience, you may print the new patient paperwork from this page and fill it out prior to your appointment.

If the appointment is for a child under 18-years-of-age, a parent or guardian must be present at the first appointment in order to sign consent forms and meet with the therapist in order to give background information on the child. If you are a legal guardian, please also bring your legal guardianship papers to the first appointment. A copy will be kept in the child's chart.
